This form is available for anyone who meets the requirements outlined in the Apprentice Program guide to submit their name to the local association for entry into the apprentice program.

This form is to enable any coach who meets the minimum requirements outlined in the Apprentice Program Guide to forward their names to the BCLA office. The BCLA office will add the coach to the mentor list & forward the selected mentor coach’s name and contact information to the applicable governing body requesting the assistance.

This form is to enable any team (coach) to apply directly to their local governing body for the use of an apprentice(s) for technical assistance. The receiving coach should ensure the responsibilities of all parties are understood as outlined in the Apprentice Program outline.
It is the responsibility of the local governing body to set the deadline on the application form to ensure the apprentice(s) are privy of the complete team season, including pre & post season schedule.
